Sodium Choloride 0.9% 3ml syringe

• Trade name: Normal Saline
• Generic name: Sodium Chloride 0.9%
• Administration Instruction: It is usually given intravenously.
• Drug classification: Minerals and electrolytes
• Indications/Use:To maintain fluid and electrolyte balance in the body, also used as priming solution in hemodialysis procedure.
• Action/Effect: None.
• Main side Affect/Adverse reaction:

1. Febrile symptoms, infection at the site of injection, venous thrombosis or phlebitis, extravasation and hypervolemia.
2. This medicines should be used with great care in patients with congestive heart failure, severe renal insufficiency, and also in conditions where edema exists with sodium retention.
3. Precaution must be followed when sodium chloride is administered to patient with corticosteroids.
4. There are no known contraindications.

• Nursing Intervation:

1. Check weight regularly
2. Regularly blood test to be done to know the electrolyte values.
3. Observe patient for any complications when administering the medicines such as vomiting, cardiovascular collapse, seizures.
4. Drink water upto 2L per day

Vitamine D 2000 unit

• Trade name:Drisdol, Calciferol
• Generic name: Vitamin D
• Administration Instruction: It is usually given orally as capsules or tablets.
• Drug classification: Vitamins and fat soluble
• Indications/Use:Used in the treatment of hypoparathyroidism, refractory rickets and hypophosphatemia.
• Action/Effect (what is expected patient response to MOA): No short term and long term effects.
• Main side Affect/Adverse reaction (any significant medical alert, warning, precaution, drug interactions, contraindications):
  • Arrhythmias
  • Confusion
  • Constipation
  • Dry mouth
  • Headache
  • Contraindicated in case of Gastro intestinal disorders, Liver disorders, biliary diorders.
  • Warning involved is donot take this medicine if you are allergic to it.
  • Caution involves discontinue the medicine if patient becomes more hypercalcemic.

• Nursing Intervation:
  1. Lab test: Conduct periodic test for serum calcium, vitamin level etc.
  2. Monitor of manifestations of hypercalcemia.
  3. Observe patient for any complications when administering the medicines.
  4. Do not breast feed when taking this drug.

Heparin 5000 unit

• Trade Name: Hep 25, Hep Lock, Hep Pak
• Generic name: Heparin
• Administration instruction: It is administerd subcutaneously or intravenously.
• Drug classification:Anticoagulant
• Indications/use: Treatment of venous thrombosis, pulmonary embolism, acute and chronic coagulopathies.
• Action/effect (what is expected patient response to MOA): Exerts direct effect on the cascade of clotting by increasing the inhibition of heparin cofactor thereby conerting the prothrombin to thrombin.
• Main side Affect/Adverse reaction (any significant medical alert, warning, precaution, drug interactions, contraindications):
  • Donot take this medicine if the patient is allergic to it,or if there is severe decrease of platelets in the blood, or uncontolled bleeding.
  • Take precaution when there is infection of the lining of the heart, uncontrolled high blood pressure, bleeding, liver disease.
  • Side effects involve bleeding, nausea, vomiting, loss of apetite, unusual tiredness, mild pain, redness, warmth, bluish coloured skin.
  • Contraindicated when there is fever chills, problem in breathing,

• Generic Name:Amlodipine
• Administration instruction: Administered orally with or without food.
• Drug classification: Calcium channel blockers
• Indications/use: Used to treat high blood pressure, and also to treat certain type of chest pains, myocardial infarction, heart failure.
• Action/effect (what is expected patient response to MOA): It produces peripheral artirial vasodilation and coronary dilation.
• Main side Affect/Adverse reaction (any significant medical alert, warning, precaution, drug interactions, contraindications):
  • Side effects include dizziness, light headedness, swelling in the feet, irregular pounding of heart, fainting and some times chest pain.
  • Contraindicated when hypersensitive, hepatic cirrhosis, bradycardia, second or third degree heart block.
  • Precaution to be taken if the patient is allergic to the medicine.
  • Donot share this medicine with others.
  • Incase a dose is missed out, take it when you remember it.
  • Store at room temperature away from light and moisture.
