1.Three different precaution used in clinical practices are-
A)standard precautions
Including,use of personal protective equipment eg.gloves,masks. Eg before any procedure eg.blood transfusion.
hand washing before and after procedure.
Safe injection practices including disposing syringe and needle after each use.
B)Safe handling of disposable contaminated articles eg.disposing waste properly like blood stained bandages in yellow bag.
C)contact precaution :
diseases or germs that can be spread by touching the patient or items present in the room of patient. Eg wearing protective equipments in case of covid-19 patient.
2)Following aspects of care must be provided as after the restraint is applied-
initial monitoring is done.
At least for 15 minutes for the first hour by a licensed practitioner or by the qualified registerd nurse.
At least every 4 hours for adults if the patient is stable
Every 2 hours for children from 9 to 17 years of age
Every 1 hour for children less than 9 years of age.
Nurse should monitor these following parameters:
Physical status of patient ,including vital signs,any injury or physical discomfort.
nurse holds the responsibility of safe and correct place of restraints and reapplication of restaraints.
Monitoring psychological and emotional status and mainitaing of dignity,safety and patient rights.
performing range of motion exercises to strained body parts, frequent turning and repositioning of patient.
Providing all other physical needs such as toileting,hydration,nutrition etc.
3)orders must be renewed every four hours for adult patients.In addition,within one hour of applying restraints,the patient must be assessed by licensed personnel.
Restraints are only used for as long as is necessary and the patient is monitored throughout this duration.It can not be used to punish a patient.
