A patient is transferred to a long term facility. this patinet may be transferred to long term facility due to cognitive or physical impairmnent or has chronic illness, needs medical, social, supportive care and basic care. because they cannot care themselves, needs extra attention and observation.
He may be transferred due to folloing reasons
1. he may need extra attention
2. he may lives alone in the home
3. he cannot care themselves
4.his care could not meet by family members
5. he has no safty at home
6. he need continuation of medical care
long term care facilities provides by some facilites namely villages, home care by either agencies or family memebers, hospice community heslath services, adult health day centers,Respite care, nursing homes.
