Intake and Output of the client . This is the process that specificaly involves the recording of all the fluid that goes into the client body and the fluid that will specificaly leaves the body of the individual . In an individual if the body is function normal then the intake as well as the output should be equal in 24 hours. This has the clinical significance in diagnosis of disease conditions and prognosis

The output of the individual and the intake gauge fluid balance will help to give a Very valuable data on the vlieny condition. It will help to identify whether the patient has specificaly undergone any kind of surgery or if the individual is having any medical condition or he is takes in some drug that specificaly can affect the fluid intake or loss in the individual

Accurate measurement as well as the documentation of the intake and the output of the individual is important because the drugs , intravenous as well as the fluid administration , the dietary decisions of the client and the tube feeding of the physician orders all these are based on the I&O of the 24-hour total . Specially this number will provide the real-time data which will help to guide the daily care of the client .
