1) Protein is required for maintaining the muscle mass and tissue repair.

If patient is on dialysis, then there is no need of restriction of protein intake. Person can take as per requirement of the body.

But if patient is not on dialysis, then we may need to restrict protein intake.

Restriction of protein intake may help in curing uremic symptoms and slows down the progression of renal failure and reduce glomerular damage.

2)

High phosphors diet may lead to bone decalcification and hyperparathyroidsm. So High phosphorus diet should be avoided.

Increased intake of potassium may lead to hyperkalemia. This is a serious life threatening condition and patient may have arrhythmiasand sudden cardiac death.So it should be avoided.

4)

It is fine to takeDairy products 1cup per day. But if it is consumed more, patient may have hyperparathyroidsm and decalicfication of bones because dairy products are rich in phosphorus.

so, options A and D can be included in nursing intervention.
