The nurse is caring for a client with end-stage renal disease (ESDR). Which of the following findings should the nurse follow up first?
Ammonia odor of the breath

In, end stage renal disease (ESDR) means the client is in stage 5.......where only15% or less then 15% of kidney remains functional means Glomerular filtration rate is become less than 15% ....for that case kidney become unable to maintain water and electrolytes balance in urine. As a result the urine output decrease. Urine can not excrete properly so swelling of leg,, foot can be seen. Along with that ammonia odour is present in breath when the stage is already prograssed.And as the client is already under endstage kidney disease , other causes like prostate enlargement , dehydration , urinary tract infection can be excluded.
Ammonia odor in breath is appear in kidney diease when the kidney unable to excrete uria but urine have to come out in another way so , uria change to ammonia and breath is one of the route of excretion . But it is seen when the kidney failure stage persist for a period of time. Another causes like , lupus, sinusitis can also be the cause.
Nausea is common for all diease where loss of appetite present .
Numbness and tingling arou d mouth is totally irrelevant because numbess can be due to cold sores, allergies , stroke,lupus.
