PRE-OPERATIVE
CARE
Pre-operative care refers to the health care provided before a
surgical operation. The aim of pre-operative care is to do whatever
is right to increase the sucess of the surgery.The nursing
assessment in pre-operative care is that of advocate who identifies
the patient's needs and risk factors that may be affected by the
surgical experience.The following are the physiologic assessments
necessary during the preoperative phase
:ASSESSEMENT
- Age
- Assess mouth for dental caries, dentures and partial
plates.
- Assess the patient's usual level of functioning and typical
daily activities to assist in patient's care.
- Infection
- Respiratory status
- Drug and alcohol use
- Nutritional status and needs
NURSING INTERVENTION
- Explore the client's fears worries and concerns.
- Encourage patient verbalization of feelings.
- To ensure that the client's understands the nature of his
treatment including the possible complications and
disfigurement.
- Monitor for the patient ability to perform the daily needs,if
necessary assist them in performing their daily activities,avoid
unnecessary falls
- Resrtict the patient to stop smoking before surgery
- Administer preanesthetic medication as ordered and keep the
patient in bed with the side rails INTRAOPERATIVE
CARE
The intraoperative care phase extends from the time the client
is admitted to the operating room, to the time of anesthesia
administration, performance of the surgical procedure and until the
client is transported to the recovery room or post anesthesia care
unit.
ASSESSMENT
- Monitoring vital signs
- Oberve the patient symptoms and record it
- Monitor for patient numbness or reduced feeling in part of
their body
- Montior for the side effects of anesthesia such as
hypothermia,nausea and vomiting
- Assess the conscious level of the patient
INTERVENTIONS
- Patient safety should be observed at all times.Prevent
accidental fallings by providing side rails
- Make certain that equipments are safely functioning
- Ensure that supplies and materials are available for use during
surgical procedures.
- Montior the patient throughout the operative procedure to
ensure the person's safety and well being.
- Maintain the strict aseptic techniques while performing the
procedure to the patient.
- Provide adequate postion to the patient.
POST-OPERATIVE
CARE
The post-operative phase of the surgical experience extends from
the time the client is transferred to the recovery room or post
anesthesia care unit to the moment he or she is transported back to
the surgical unit, discharged from the hospital until the follow-up
care.
ASSESSMENT
- Assess air exchange status and note the patient's skin
colour
- Assess the level of consciousness
- operative site examination
- Verify patient identity
- Assess the patient fluid status
INTERVENTIONS
- Fall precaution implementation by making sure the side rails
are raised and restraints are secured well
- Provide easy access to call bell and commode
- Avoid restraints,and if possible allow family member or staff
always asked to sit with them
- Proper sterile techniques should be followed while doing
dressing
- Assist the patient in early ambulation.check the postoperative
activity orders before get-ting patient out of the bed.Then have
patient sit on the edge of bed for a few minutes initially
- Administer the medications as ordered by the physician.
