Real Life Clinical Reasoning Scenario - Type 1 Diabetes Mellitus ISBAR.
ISBAR means a form of communicating a message to physician it means
Identification situation, background, assessment and recommendation
ISBAR report on diabetic mellitus
I= name of nurse, ward where you are working, pt's name, MRD and diagnosis
S= patient is complaining of blurred vision, increased thirst, tremors in hand , so please come a see the patient.
B= patient had diagnosed with type 1 diabetes in a childhood and after that pt is on several medications the disease was under control but from few days ago pt developed the symtoms like tremors in hands ,dry mouth, increased thirst and then was admitted to hospital and now pt is complaining of such symptoms again
A= i think pt is developing hypogycemia as these symptoms like blurred vision , tremors in hands, excessive thirst are due to hypoglycemia , I assessed the pt and checked the blood glucose level which was really low. I started the fluids now
R= I think we should test the blood of the pt for HbA1c which could help in treating the disease well and also we must increase the insulin dose and diet therapy to the pt and educate them when should he can eat and do exercise. Sir please come a do the further assessment of the pt
