CLAIM LETTER ASSIGNMENT
HEFTY TRASH BAGS
While shopping at Walmart, you purchased 4 boxes of Hefty Tall Kitchen Garbage Bags, each containing a $5 rebate for which you were eligible. Upon arrival back home, you completed the rebate forms and mailed them with the required proofs of purchase and receipt; three weeks later, your $20 rebate check arrived. Unfortunately, you got busy and promptly forgot about the rebate check---until 3 months later when you found it while looking for a postage stamp.
Upon presenting the check to cash at your bank, you’re told the check is past it 90-day limit. Thus, you decide to write a claim letter requesting a replacement check from the rebate center, YA, Inc., whose address is 10 South 5th Street, Minneapolis, Minnesota, 55402. Of course, you will enclose the original, expired rebate check to help expedite the replacement process. You also investigated and decided to mail your letter directly to the person at YA, Inc., that is in charge of the customer service team: Kathleen V. Cowden.
Now, write the claim letter using block style with mixed punctuation.
Note: Walmart, Hefty, and YA are three distinct companies. Walmart is where the product was purchased; Hefty is the brand name of the product purchased; and YA is the company who handles the entire rebate process.
In fact, YA handles rebates for hundreds of companies and processes rebate requests for thousands of products sold by these companies. Thus, it is in the interest of YA to keep customers satisfied.

Sub= request for the reissue of the rebate cheque
Hi,
Best wishes of the day. I am writing this mail to bring in your notice that recently I bought 4 boxes of Hefty Tall Kitchen Garbage Bags from Walmart each containing a $5 rebate and thus I am therefore eligible for a total rebate of $20. In fact, I received the rebate cheque from your side before three months but unfortunately, I got stuck in some personal assignments and therefore forgot to deposit the check for cash in. When I presented this cheque recently, the bank informed me that the cash-in period of the cheque i.e. 90 days from the date of the cheque has been expired and thus I cannot have the encashment of the cheque.
I, therefore, request you to kindly reissue the rebate cheque or the amount of $20. I am enclosing the original cheque of $20 which I received from your side. I am quite hopeful and optimistic that you would process my request at earliest.
