NURSING CARE PLAN FOR HYPERTHERMIA

Fever /hyperthermia / pyrexia, is characterised by  uncontrolled rise in the body temperature,above 37.5 degrees Celsius.
Common etiologies include ongoing infection, trauma, exposure to hot environment, and increased metabolic rate.

Nursing Plan for Hyperthermia due  to infection

GOAL

To maintain normal body temperature.

Nursing Interventions

•   Assessment of  patient for vital signs ( Pulse rate, Respiratory rate)
• Providing cool and calm environment to the patient.
• Adjust the room temperature and Provide ventilation by fan or opening doors and windows.
• Remove excessive clothing, blankets and linens.
• Cold sponging  to the patient, if temperature is consistently high.
• Provide aseptic care and follow infection prevention protocols to prevent infections.
• Administration of prescribed Anti pyretic and antibiotic / antiviral medication.

• Elevate head end of bed to facilitate comfortable breathing.

• Start preparing a fluid balance chart for monitoring input and output if required.

• Recheck Vital signs atleast every 4 hours.
