Normal Assessment

Assessment: It contains both Subjective and objective data

Subjective data: patient says about any abnormal changes

Objective data: nurse observes

Assessment of head:

• Assess the patient status
• Check the color of hair, scalp for any dandruff, lice or lesions
• Observe for size and shape of skull
• Palpate the head via the fingers to check for any tenderness
• Check the features of face

Assessment of neck:

• Assess the symmetry of neck
• Check for any lumps are present
• Palpate the neck for any tenderness or pain
• Perform range of motion to check the movement of neck in all directions like left, right, up, down and rotate

Assessment of lymphatic:

• Palpate for any internal and external lymph nodes
• Check for edema

Assessment of eyes:

• Check the general appearance of eyes like color, symmetry
• Inspect the vision like visual acuity test by using snellens chart
• Observe for any redness, itching, discharge or tearing
• Assess for pupillary response: reaction to light, equal size of pupils
