Subjective:Breathing Difficulty

Objective: Crackle breath sounds

Nursing Diagnosis: Ineffective airway clearance related to thick tenacious secretions and poor cough

Short term goals:

Client will able to:

Maintain normal respiratory rate

expectorate secretions and mucus

cough effectively

perform chest physiotherapy effectively

drink atleast 100ml of liquid for easy expectoration

Long term goals:

To Maintain clear airway

To maintain easy breathing in lying position

Nursing intervention:

Assessed respiratory rate with the assessment that use of accessory muscles for braething

Provided semi-fowlers position

Encouraged deep breathing

Encouraged coughing

Administered expectorants to remove the thick secretions

Provided chest physiotherapy to facilitate expectorants

Taught pursed lip breathing, cough and breathing sxercises

Evaluation:

Client maintained clear airway

Client maintained normal respiratory rate

Client have not used accessory muscle for breathing

Client expectorated secretions

Client lung sounds were normal with no crackles or wheeze.
