I need a care plan for COPD. In this care plan i need you guys to include nursing diagnosis for COPD. Then 6 short term and 6 long term goals for COPD. I need one nursing intervention for each each short term and each long term goal. And the evaluation for each nursing intervention.

Diagnosis for COPD:
Disease associated with COPD such as lung cancer, anxiety, depression, heart problems, arthritis, bone disease should be taken care and treated on time as such events can lead to more mortality rate.
The aim of COPD is to evaluate the prognosis, diagnosis, risk factors, and future aspects of the disease in order to prevent disease and give proper therapy guidelines.
Patient who has symptoms related to dyspnea, extreme cough and sputum production should be tested according to COPD guidelines.
Management of COPD:
1. Evaluating patient on the basis of symptoms and predicting the future risk for the condition.
2. Giving education to person who has smoking habit to quit as soon as possible.
3. The main task is to reduce the number of cases and the future risk associated with COPD.
4. The scope of COPD includes taking advantage of both pharmacological and non-pharmacological area in order to provide benefits to patients.
Prevention of COPD:
1. Spreading awareness for quitting smoking habits and the risk associated with it.
2. Influenza vaccination is reducing the incidence of respiratory tract disorder.
3. Long term oxygen therapy is improving the health of people associated with chronic disorder.
4. Inhaler technique use is utilized more for breathing related issues.
