I need a care plan for a patient who suffers from hypertension. Please list nursing diagnosis, desired outcome, nursing intervention and evaluation of patient.
1) Diagnosis
Risk for decreased cardiac output releated to increase in vascular resistance as evidenced by continous hypertensive readings .
Desired outcome / goal
To maintain the blood pressure in a individually acceptable range
intervention
Assess the risk factors as well as condition that stress the heart
Educate client to participate in activities that reduces blood pressure or cardiac work load
Educate relaxation techniques ,guided imagery and ,distractions
Educate the technique of self monitoring of BP
Administer medication as advised by the physician eg : beta blockers
Evaluation
Client is maintaining the blood pressure with in the normal range limit
2)Diagnosis
Activity intolerance releated to generalized weakness as evidenced by abnormal hear rate or Blood pressure response to physical activity
Desired outcome / Goal
To demonstrate a decrease in physiological signs of intolerance
Intervention
Assess the factors which are responsible for generalized weakness
Monitor the patient response to activity by noting pulse and blood pressure
Educate energy conserving techniques to the client eg: chair while showering
Encourage progressive activity and self care when tolerated
Evaluation
Patient reports a measurable increase in activity tolerance
3) Diagnosis
Knowledge deficit releated to disease process and treatment regimen as evidenced by denial of diagnosis
Desired outcome / goal
Client should verbalize knowledge of disease process and treatment regimen
Intervention
Identify patient readiness and ability to learn
Encourage patient to ask frequent question
Provide patient with information concerning disease process including risk factors, complication and treatment regimen
Evaluation
Patient develop knowledge regarding disease condition and asking questions for clarification .
