My ​​​Nursing care plan - A nursing care plan provides direction on the type of nursing care the individual /family/ community may need. The main focus of nursing care plan is to facilitate standardized, evidence based and holistic care . A care plan includes the following components : assessment, diagnosis , expected outcomes , interventions, rationale and evaluation.

• To promote evidence based nursing care and to provide comfortable and familiar conditions in hospitals or health centers.
• To promote holistic care which means the whole person is considered including physical, psychological, social and spiritual in relation to management and prevention of the disease.
• To record the care.
• To measure the care.
• To provide treatment measure health issues or disease conditions.
• To ensure the psychological support and reduce stress anxiety to the patient.

Components of care plan :

• client assessment , medical results and diagnostic reports. This is the first step in order to be able to create a care plan.
• Expected patient outcomes are outlined including long term and short term.
• Nursing interventions are documented in the care plan.
• Rationale for interventions in order to be evidence based care.
• Evaluation . This documents the outcome of nursing interventions.

Nursing priorities: there are 3 main priority for nurses.

1. ABCs + V&L

Airway problems, breathing problems, cardiac or circulation problems , vital signs concerns, lab values that are life threatening.

2. Things included in priority 2 -

Changes in mental status , untreated medical problems like DIABETES who hasn't had insulin, pain , urinary elimination problems .

3. Priority 3 -

Health problems not included in first and second categories like rest and activities.
