Mr. Rodriguez is noted to have a reduced inspiratory and
vital capacity.
A. On the Bird IPPB machine, which control(s) could be adjusted to
increase the volume achieved during therapy? System pressure and
flow rate.
B. How would you adjust the setting of these control(s) for this
purpose?
C. If the Bennette machine is used what controls can be adjusted to increase the volume? How would the controls be adjusted?
A. Intermittent positive pressure breathing or IPPB is used for this patient ,this machine is a ventilatory apparatus in which the compressed gas is delivered under positive pressure into patient's airway until a preset pressure is reached. Passive exhalation is allowed through a valve. For this patient ,nebulizing medication is prescribed and also specific pressure and volume amounts are adjusted in the machine.The patient stimulates inspiration,with positive pressure support,and followed by airway pressure returning to atmospheric pressure with passive expiration.
B. Flow and pressure will need subsequent adjustments to patient's needs and goal. The therapy is volume oriented.Therapists would set up the machines by dialing appropriate sensitivity,flow and pressure levels.The flow level determines how much pressure was given to patient.Sensitivity determines how hard the patient had to work to trigger a breath.A circuit was then connected to mask and a nebulized medicine is added to the circuit via nebulizer cup which is connected to a flowmeter via oxygen tubing.
C. If Bennette machine is used a mask is connected to patient, which is connected with a valve to a cylinder of compressed air is used.This device supplies controlled respiration with positive pressure to patient and also this device was not controlled by electricity, so it is not danger for the patient.
