1. 1 ROLE AND 2 RESPONSIBILITIES OF SERVICES DELIVERY WORKER INVOLVED IN THE PLANNING OF SERVICE IN AGEING SUPPORT.
A. Persons assessor
Assessor means any person who contributes to a trainee’s assessment in the workplace, providing feedback to the trainee and to the clinical supervisor and educational supervisor.Aged Care Assessment Service (ACAS) assessor will be a nurse, social worker, occupational therapist or other health care professional
Role:Assessor role includes assessments of their performance against the Quality Standards through comprehensive audits and assessment contacts. These assessments are undertaken by registered quality assessors.
Responsibilities: .They will visit in home or in hospital to ask about how are managing from day-to-day and about overall health situation.
They will respect the in-house rules of any organisation.
They will maintain competence and knowledge of contemporary practice.
B. Carers
Carers is a family member or paid helper who regularly looks after a sick elderly, or disabled older adult.
Role: Carers can help elderly adult to be as independent, healthy and well as possible.
Responsibility: Carers responsibility is
helping elderly stay connected to their local community, or be physically and mentally active, or eat nutritious food.
Some carers might have to help older adult with tasks like banking, shopping and housework where the person is quite independent.
Other carers may have a more intensive care role helping with all daily living tasks – feeding, bathing, dressing, going to the toilet and taking medicines.
C. Support workers.
Support workers are the care assistants on a day to day basis who help olderly people who need assistance with their personal care or daily activities.
Role; Support workers are vital part of providing vulnerable people with care and to improve quality of life of elderly adult.
Responsibility: Providing practical support for elderly individual and family.
Helping household tak, personal care and paperwork of older adult.
Supporting and monitoring older people in their healthcare needs.
D. Primary service provider
Primary health provider is the main health care provider in non emergency situation.
Role: Provide preventive care and teach older adult healthy lifestyle choice
Responsibility: Consult with older patients, discuss their health care needs, and offer advice to them.
Give health education and councelling older adult all the health care needs to improve their quality of life.
Prepared health care plan as needed
Attending ongoing training and courses to advance knowledge and teach elder adult to improved their health.
E. Health professional 1 ; Geriatrician:
A geriatrician is a doctor who specialises in health care of the older people and disease that affect them using a team approach.The approach tends to be holistic and involves multidisciplinary team.
Role: Plays the major role in the treatment of the elderly living at home, or in nursing homes.
Responsibility: A geriatrician is a coordinator for elderly patients who have physical and mental disorders. Geriatrician take care of not only diseases, but also the patients' lives after discharge from hospital. Geriatrician should coordinates a team approach and maintain contact with care managers.
Geriatricians provide a patient-centred, compassionate and holistic approach. They work very closely withother hospital doctors, social workers and many others involved in caring for elderly people including families and carers. Diagnosis and management of acute illness as well as chronic disease, disability and frailty is all part of geriatician responsibility.Providing rehabilitation after an illness is a key aspect of the work, and geriatricians will work with other team members to enable patients to work towards specific rehabilitation goals. Developing a good knowledge of physical treatments and adaptations and aids is important.
F. Health professional 2; Geriatric nursing;
Geriatric nursing specialized in the care of older and elderly adult.
Role: Geriatric nurses play important role in addresses the physiological, developmental, psychological, socio-economic, cultural and spiritual needs of an aging individual.
Responsibility: As a geriatric nurse, you will be required to perform a number of duties. For example, you will often be responsible for
Measuring and recording vital signs
Administering medications
checking blood pressure
changing wound dressings
caring for ycatheter
managing continence.
Exercising and massaging patients
Watching for signs of elder abuse
Transporting patients to doctor’s visits and other appointments
Helping patients with their daily needs, such as bathing, dressing, and using the bathroom
G. Service delivery workers 1; Geriatric physiotherapist
Geriatric Physiotherapy or rehabilitation is deals with managing physical problems in elderly people.It mainly focuses on people in the process of ageing.
Role: Geriatric physiotherapy role is to helps aged people to improve physical balance and strength, build confidence and keep them active.
Responsibilities: Geriatric physiotherapy responsibility involves a sequence of preventive and intervening measures to provide the care and support essential to improve and manage functional independence and the quality of life of elderly adult.
Old people offers to improve mobility & balance, build strength, boost confidence in their physical condition, boosts joint range, and enhances strength & flexibility which improve their health status.
H. Service delivery workers 2; Geriatric nutritionist
A geriatric nutritionist is a dietitian who specializes in nutrition for the elderly. They primarily work with older adults and know how to customize nutrition interventions to meet the need of aging seniors.
Role: Geriatric dietitian play a key role in preventing and managing of nutrition related disorder in older adult.
Responsibilities; Nutritional assessment of older adult.
Malnutrition risk screening of elderly patients
Formulate nutritional care plan
Provide nutritional therapy by providing adequate and approprite diet scedule.
I. Other service provider.1.Non direct elderly care roles
There are many workers who are not directly involved in caring for old age adult, but who do play a big part in taking care of old aldult.
Roles;to make sure that elderly people have everything that elderly peoples need.
Responsibility: These non-direct care roles include residential care managers, caterers,cleaning companies, laundry staff and gardening and maintenance contractors.
J Others service provider 2. Age care volunteers
Age care volunteering is a voluntary act of an individual or group freely giving time and labour for elderly care service. Many volunteers are specifically trained in the areas they work, such as medicine, education, or emergency rescue.
Role; Age Care Volunteers play an important role throughout aged care system, especially in regional and rural areas.Helps older adult to reduce stress, improve mood, help prevent loneliness and lower the risk of developing high blood pressure
