Patient history , patient appearance,vital signs , wieght, hieght,blood sugar levels

Focused assessment of face , neck ,skin

Assessment for skin -

Take a thorough history of patient skin care about bathing, shampoo, topical products

Skin appearance with seasons ,nail thickness, colour change

Allergies to other medications

Physical assesment to skin colour, temperature, moisture,lesions

Lesions are macule, papules, plaque, nodule ,cyst , vesicles , scales ,crust

Look out for dry skin

Assessment of head

Inspect face - inspection of eye', eyelids, conjunctiva ,sclera -

Check for

Eyelids swelling , yellowish sclera, conjunctiva swollen

Eyes - strabismus,aniscoria ,

Reactive to light ,

Ear - tenderness, swollen mastoid ,ear wax , hearing test

Nose - symmetrical, drainage , lessions, olfactory response

Mouth - inspect lips, tongue, inside the mouth

Neck - inspect the trachea for lesions such as goitre

Inspect jugular vien distension

Palpate the lymph nodes

Skill utilised during assessment are - communication skill the most important one during taking history

Active listening skills - that are necessary for good concentration on work

Caring, adaptability, common sense self confidence

2. During the assessment i felt that patient was not interested in sharing the history

3. Next time while taking the assessment i should motivate about the condition of patient and improve the interpersonal relationship so that he can share the history comfortably
