Skin, hair, and nails are parts of the integumentary organ system. Skin is the biggest organ of the body. Assessing skin will help to provides helpful information about all other systems of the body. Assessing skin, hair, and nails provide nurse valuable informations about the general health status of an individual. It helps to identify the changes or health issues caused by change in environment and can helps to show indication of internal complaints in connection with other organs. When a nurse assess skin, hair and nails, changes in the them. It can help the nurse to capture early warning of a emerging health problem. But sometimes the significance of carefully assessing the integumentary system for notable changes is often ignored and it might cause complications. A detailed assessment of this system can help nurses to detect real or potential health issues or complications in the skin, hair, nails and other parts especially in underlying systems.

Rationale for assessing the peripheral vascular & lymphatic

The part of the circulatory system that consists of the veins and arteries in the arms, hands, legs and feet is called as peripheral vascular system. Proper assessment of this system will helps to identify any occlusion or damage in the blood flood. Diseases such as Peripheral vascular disease is commonly seen in some people especially in elderly people. It is a neglected condition and mostly sometimes they will not get proper diagnosis and management. Careful assessment of this system provides valuable information on over-all health condition of an individual and can help the nurse to define the current condition arteries and veins.

Similarly, assessment of lymphatic system is also crucial for nurses. It is because ,lymphatic system plays an important role in the proper functioning of the immune system .A network of tissues and organs that helps to eliminate poisons, toxins and other unwanted materials are called as the lymphatic system. The main purpose of the lymphatic system is to carry and transport a fluid called lymph throughout the entire body. Proper assessment will helps to identify whether the system is blocked or not and can helps in planning management. In the assessment, palpation of the lymph nodes gives nurses a proper information about the probability of a malignant or inflammatory progression happening in the body. It will always help to identify lymphedema.
