nursing care plan is a documented guidelines for providing individualized patient care according to the needs. a systematic nursing process used for formulating an effective nursing care plan .which consist the following steps ; assessment , diagnosis , planning , implementation , evaluation .

assessment is the first step in determining patient's health status , which consist a comprehensive physical assessment and data collection. laboratory data and diagnostic test are important patient data which can't be excluded during data collection. all data should be complete and accurate including laboratory findings in order to identify correct nursing diagnosis .

care plans will help the student nurses to involve in all process of nursing care. a student nurse should be genuine and accurate during data collection process of nursing care plan. simply writting a number instead of a patient's relevent CBC report will affect proper diagnosing and planning of care. a CBC report can provide valuble information about overall health and detect a wide range of disorders. while writting a care plan the student nurse should always go through patient lab values, in order to recognize trends that affect patient and to develop a good nursing care plan.
