Head : Ask about dizziness, headache, fainting, seizures, paralysis, weakness, numbness or tingling, burning sensation, pain when moving or walking, ability to move or walk, loss of memory, concenteration changes and behaviour changes.

Eyes: Ask about vision, any changes in vision, blurred vision, double vision, blind spot, ocular pain, photophobia etc.

Ears: Ask about hearing, hearing aids, any changes in hearing, tinnitus, dizziness, ear pain, ear drainge etc.

Nose : Ask bout any stiffness, running nose, itching, dryness, epistaxis, sinus pain, sense of smell etc.

Throat : Ask bout issues with teeth or gums, bleeding gums, any sores in mouth, blisters, thrush, tonsilitis, hoarseness of voice etc.
