1) the nursing process involves the five stages which are assessment diagnosis planning implementation evaluation. The first is assessment:-

Collection of the all data of the patient.2) gather all info information about their physiological state sociological and spritual status.3) with this collection the nurse can differentiate between the normal and abnormal condition of the patient.4 ) this collection data are of two type subjective and objective. Subjective data includes the verbal statement and the objective includes vital signs,height and weight etc.

The next is diagnosis:-

The North American nursing diagnosis association provides nurses with up to date list of nursing diagnosis. 1) nursing diagnosis is the clinical judgement about the responses to actual and potential health problem. In diagnosis check all the health problems and make a report which shows what are the abnormality.

- the next stage of nursing process is the planning:-planning is the kind of stage where clause and outcomes are formed which have impact on the patient health care. Care of the patient by the nurse is a most important in this part. Goals should be measurable, time oriented, specific , attainable etc .

Next is implementation:- is the apply of the all care, action that should be done by the nurse which have been involved in the planning part for the better health of the patient. Involves applying the monitoring machine ,oxygen machine ,direct or indirect care and proper medication.

The last is the evaluation:-in this will evaluate the all the outcome of a patient after his treatment. For the check all the reports that patient recovered or not. If sometimes the condition of patient is is is better than earlier but not well so the reassessment can also be done.

2) clustering of data is a kind of of making a group in which contains same kind of the objects or the information and the different object in different groups. It necessary to modify the data or arrange it in the sequence. So it is the arrangement for grouping the same kind of object or information.

3) open ended questions are that kind of question which are not end with simply yes or not it means their answer should be in the explanative form .while the closed ended question are short questions there answer are in yes or no.

The open ended question could be used in client assessment interview like in the patient or nurse it should be done1) how are you feeling today? how are you taking this medication? What is the routine of your daily diet.?

Other hand the close ended question should be are you feeling well ? ) are you taking your Medicine properly.? 3) are you doing some physical exercise or not?
