Psychosocial integrity is a state of balance of psychological and social aspects of an individual which is usually altered when client is having stress, health related issues and crisis which may have an effect in the emotional, mental and social wellbeing of an individual. There are many risk factors than lead to an alteration in the psychosocial integrity in an individual.
The following are the risk factors related to alteration in psychosocial integrity.
. Behaviour modifications
. Lack of self-control
. Substance abuse disorders
. Ineffective coping mechanism
. Family conflicts
. Grief and loss of a loved one
. Stress
. Ineffective communication
. Abuse and negligence and violence
. Anger or criminal behaviour

. Helplessness and shame
. Feeling of lack of self esteem
. Worthlesness and lack of responsibility
OBJECTIVE DATA:
. Sad, hopeless and anxious.
NURSING DIAGNOSIS :
. Body image disturbance related to deformities due to an accident .
NURSING CARE PLAN / IMPLEMENTATION
. Maintain a good communication with the client to develop a positive relationship hence the client will be able to express they feelings and thereby increase a positive response
. Help the client to achieve a positive feeling about self and accepting they body the way it is and by supporting them psychologically .
. Health teaching regarding the changes in the functions and explain the important of a possible attitude
. Help explore the clients weakness and strength and teach the client and the family regarding positive attitude.
EVALUTAION :
.Able to get back to normal state and perfeom daily activities
.Able to accept the physical and mental condition
.Ability to have a positive attitude
HEALTH TEACHING :
Evidence bases practice is the use of available evidence to ensure the patients safety
Using various resources related to evidence based practice enhances the nurse to provide quality and effective care to the client.
The nurse must help to meet the emotional, social and behaviour demands in life.
Based on the evidence the client may have emotional distress, hence ways to adapt to the changes and measures to enhance emotional well being is followed by providing cognitive therapy.
The behaviour disturbances of the client can be reduced by providing psychological support
The social demands in life are often the major factor that affect the client as to the way the society looks at them due to the alteration in they normal physical and psychological well-being hence measures are taken to improve the social well being of the client by encouraging the family to take a part in the development of the clients health status .helpinf the Client to have a positive attitude .
Monitor the progress of the teaching and evaluate the effectiveness of the teaching to determine if the client finds it helpful.
encourage the client to accept the way they are and also encourage the family members to work along with the client to accept the way they are.
Explain the role and importance of prosthetic devices that will aid in providing assistance to the client .
Teaching regarding personal hygiene , rest and sleep and the medications administration is often followed. .
