Medication reconciliation is, comparing a patient's medication
orders to all of the medications that the patient has been taking.
This reconciliation is done there by we can avoid medication errors
such as omissions, duplications, dosing errors, or drug
interactions. They will be done through every transition of care in
which the new medication ordered or existing order is rewritten.
Transition process contain five steps: (1) prepare a list of
current medications
(2) to prepare a list of medications to be prescribed
(3) compare the current medications list and prescribed medication
list
(4) develop a clinical decisions based on the comarison
(5) communicate the new list to appropriate family member and to
the patient.
Challenges associated with reconciliation
The medication reconciliation is a multidisciplinary effort against
the organization.it includes
1. About74.8% incomplete and inaccurate medication data can be
occur.
2. 59.7% inconsistent practices across departments their
disciplines and shifts
3. 47.9%Patients discharged with an incorrect medication list
4. 46.2% difficulty in importing external medication history,
includes home medications
5. 42.9% lengthy patient interviews, therefore calls to families,
pharmacies and providers
6. About 30.3% outdated workflows provide bad medication
reconciliation practices
