Abanga was rushed to the hospital on account of fainting, shortness of breath, palpitations and nausea. On assessment, Abanga was pulseless, with the unrecordable blood pressure but a heart rate of 450bpm. His ECG showed no P-wave nor QRS complex. His Hb was 7.0g/dL (normal range 12.5-16.5g/dL) and MCV was 70fL (normal 80-100fL). Abanga’s drug history indicated that he has been taking an antacid (aluminium hydroxide mixture) for the past three months now due to his palpitations and was on prescribed ferrous sulphate which he has been taking for the past one month due to a previous diagnosis of anaemia. An impression of arrhythmia and anaemia was made by the physician and Abanga was defibrillated with electrical shocks and started on verapamil, warfarin and adrenaline. Laboratory investigations the next day showed high serum potassium levels.

1- By the signs and symptoms it can undestand that patient is having Ventricular fibrillation and Iron deficiency anemia.
2- during the assessment Abanga had no pulse,no recordable blood preassure ,but heart rate is too high ,In ECG abscence of P and QRS complex.These are the signs directing to venticular fibrillation.
Patient is on antacids for long time,the administration of the antacids may interfere with the absorption of iron from the intesinal tract.Hense the anemia in the patient is iron deficieny anemia.ferrous sulfate is a medicine used to treat iron deficiency anemia.
3- Abanga is on antacids for the last three months.This long term intake will lead to iron deficiency in the body.Antacids prevent the absorption of iron from the intesinal tarct. Thus antacids may be a problem for anemia.
4- Verapamil is used in ventricular dysarrythmias because the drug's ability to shorten the refractory peroid and acclerate antegrade conduction within the accessary pathway.there is risk of blood clots hense the warfarin is given.Cardiac arrest can be assoisated with ventricular fibrillation,because of this reason adrenalin is given.
