The ' Perioperative Surgical Home' presents a concept of care continuum that have an initiation of Home based care , then progress to care delivered in community settings through Wellness centers, physician offices, pharmacy stores, urgent care clinic and then to hospital. Next phase of the care continuum is Posy acute phase . It is starting from hospital to Post acute care, that usually followed with home care and Rehabilitation.

Shared decision making is an important aspect in the PSH model. In the acute care phase , the decision has to be made by the anesthesiologist, physician, patient and surgeon together.
