An interdisciplinary team is a group of health care providers. It involves individuals from different fields. Its goal is to achieve a better quality of care for the patients and provide the best results.

1)all members of interdisciplinary communicate with each other through various means----

  1. Scheduled meetings
  2. Share electronic medical records
  3. Standardized patient assessments
  4. Secure e-messaging

As this type of team has many members, those who have many responsibilities so couldn't make ground rounds daily. meetings will be scheduled as per the need.

Frequency can be varied with the seriousness of the theme the team is working on. As only meetings need to be scheduled otherwise e-messaging or sharing records can be communicated among them easily.

2)fol follow members of the health care team are present:-

  1. Physicians
  2. Nurses
  3. Case manager
  4. Social work
  5. Occupational therapist
  6. Dietician
  7. Patient care assistants
  8. Psychologist

It's effective because the Presence of all of them is important for a better understanding of the problem and an easy and fast solution. Health issues need to be solved by involving every person. Doing this will help in better and fast results. And the problem will be solved at all levels.
