The nurse client relationship is the foundation of therapeutic process. It is a relationship in which both participants must recognize each other as unique and important human beings. It is also a relationship in which therapeutic learning process occurs.

Hays and Larson describes tge role of nurse as provide the client opportunities to :

* identify and explore problems in relating to others.

* discovering healthy ways of meeting emotional needs.

* experience a satisfying interpersonal relationship.

Caring attitude and behaviors includes

1. Therapeutic communication

2. Client centered care

3. Maintaining boundaries

4. Protecting the client from abuse.

1) Therapeutic communication:  

Therapeutic communication is an interpersonal interaction vbetween the nurse and the patient during which the nurse focuses on the patient 's specific needs to promote an effective exchange of information.

Principles of therapeutic communication.

- the patient should be primary focus of interaction.

- professional attitude sets the tone of therapeutic relationship.

- use of self disosure cautiously and only when therapeutic purpose

- avoid social relationship with patient.

Therapeutic nurse client relationship :

An interaction between two peple ( usually a care giver and a care receiver) in which input from both participants contribute a climate of healing, growth promotion and / or illness prevention.

Components of TNCR:

# Rapport: it is a relationship especially when the nurse showes a willingness to become involved with another personto grow towards mutual acceptance and understanding of individuality. The nurse establishes rapport through demonstration of understanding, warmth & nonjudgemental attitude.

# empathy : an ability to feel with the patient while retaining the ability to critically analyze the situation. It is an ability to put oneself in other person's circumstances and feelings. The nurse need not necessarily have to experience it but has to be able to imagine the feelings associated with the experience.

# Warmth: it is the ability to help the patient feel cared for and comfortable. It shows acceptence of the each person 's uniqueness.

# genuineness:

Involes being one's own self. This implies that the nurse is aware of her thoughts, feelings, values and thei relevance in the immidiate interaction with the client.

Dynamics: therapeutic use of self, self awareness, the Johari window.

Phases:

A. Pre-interaction phase: this phase begins when the nurse is assigned to initiate a therapeutic relationship and it includes all that the nurse thinks, feels o does immidiately prior to thrle first interaction and exploration.

Tasks of nurse: explore own feelings, fears. Analyze owb professional strength . Gather data about the patient. Plan for the first meeting with the pt.

B. Orientation phase: here the nuse 1st tym meet with the patient. One of the primary concerns is to find out why the patient sought help.

C. Working phase: here the nurse and patient explore relavent stressor and promote the development of insight in the patient by linking perception, thoughts, feelings and actions & provide him the opportunities for independent functioning. Evaluate problems and goals and redefine as necessary.

# termination phase:

The most difficult but most important phase . Here the nurse evaluate the experience of relife of the patient , mutually explore the reality of separation and deals with the feelings emerged with it.

3. Maintaining boundaries:

There are some professional boundaries when it comes to the nursing .

• self disclosure: self disclosure on the part of may be appropriate when it come to ttherapeutically benifits of the client. It should never be under taken for meeting personla need of the nurse.

• gift giving: it can be a therapeutic process on care reciver's part but accepting financial gift is never aproprite for a nurse.

• touch : nursing involes the concept of therapeutic touch to provide comfot to the patien but in case of the patient with high anxity the permission should be taken first and explained the purpose .

• friendship or romentic association: the relationship beteen the nurse and client should highly professional and should move toward a appropriate termination. If a nurse is not able to follow tge professional ethics he or she must withdraw the nurse client relationship.
