In patient-centered care model,the patient can make decisions about their own care and treatment. Clinicians are there to inform, advise and support, but it is ultimately up to the patient to determine what course of action they will take.
The goal of patient-centered health care is to empower patients to become active participants in their care. This requires good communication skills and address patient needs effectively. The overall effect is that it improves the productivity and quality of care
Teamwork & Collaboration:
Collaboration and teamwork require a mix of interpersonal, problem solving, and communication skills needed for a group to work together towards a common goal.
Working in teams enables employees to share knowledge, work more efficiently and effectively. It promotes healthy employee relationships. Healthy employee relationships lead to better team performance and overall productivity. Hence it improves the patient satisfaction and quality in services.
Evidence-based Practice (EBP):
EBP is a problem-solving approach to clinical decision-making within a health care organization. It integrates the best available scientific evidence with the best available practical evidence. EBP
helps clinicians stay current on standardized, evidence-based protocols, improves transparency, accountability, and value of care given, improves quality of care and improves outcomes.
Quality Improvement (QI):
Quality improvement (QI) is the framework that is used systematically to improve the ways the care is delivered to patients. Processes have characteristics that can be measured, analyzed, improved, and controlled.It focus on the system of care in which they practice. So the good systematic approach of caring will improves the outcome and quality of care
Safety:
Patient safety is an important aspect of high-quality health care. It aims to prevent and reduce risks, errors and harm that occur to patients during provision of health care. Patient safety is fundamental to delivering quality essential health services.
Much of the work defining patient safety and practices that prevent harm have focused on negative outcomes of care, such as mortality and morbidity. Nurses are critical to the surveillance and coordination that reduce such adverse outcomes.
Informatics:
In the changing world health informatics have an important role in affecting the outcome of services. The computer system keeps the health records of the patient and it helps to give the care in proper time without any delay. All informations regarding health are stored in computer.
Information technology can improve the quality of care patients receive by averting medical errors, improving communication and boosting efficiency.
