Pt is a post partum 29 year old - G5P5
NURSING DIAGNOSIS:
PPD related to complications during pregnancy and difficulty latching for the baby - pt spoke to a a latching consultant and the issue is fixed. Pt has stated that taking walks has helped with stress.
At risk for nutritional imbalance related too forgetting to eat - pt has been setting timer to remind
elevated temp and WBC - at risk for infection.
1. Patient Education:
Patient should attempt only to shoulder those responsibilities that she can handle. She should be encouraged to communicate every feeling to close ones and do not isolate themselves. They should have some personal time for themselves and take the right amount of rest and sleep. Family members should participate in providing emotional support to the patient. Explain to the patient to avoid consuming foods that increase depressive symptoms like alcohol. Patient should eat a balanced diet and exercise daily allowing the body to heal faster. Massage , meditation, a hot bath and other mindfulness practises like reading a good book helps the patient to feel better. Advise patient to make an action plan and write it down and follow it daily. Encourage patient to engage in activities which they enjoy. Advise patient to maintain strict hygienic practises to prevent infection. Maintain diet high in nutrients like vitamin C, but low in processed food.
Discharge Planning:
Patient should be taught regarding the consumption of medications prescribed by the doctor like antidepressants to decrease depressive symptoms and not to discontinue it until her doctor orders. Advise patient to take medicine at correct time daily. In case of any side effects , report immediately. Help patient in engaging in women support group within the hospital. Arrange facilities for providing psychotherapy for the patient.
2. Stage 6 of Erickson's Psychosocial Development: Intimacy Vs Isolation (20-39 years- Early Adulthood) In this stage the adult must develop an intimate relationship or else suffer from feelings of isolation.
