Interdisciplinary primary care teams have a key role in identification, treatment and prevention of diabetes along with the best routine care for the effective management of the condition.
It is seen that the interdisciplinary diabetes care teams act as the interface between primary & specialty care in association to offer the improved care outcomes.
Diabetes mellitus is a complex metabolic disorder in the body which requires a complete care plan for its control and its feasibility and effectiveness depends on the coordination of the interdisciplinary diabetes care teams, including the physician, pharmacist, nurses and other associated health professionals. A proper diet plan is having a crucial role in the management of the diabetes. Then there is need of the proper exercise in this condition hence the physical trainer to manage this condition.
At the same time there is need of the education and training programmes offered by the interdisciplinary teams for the effective control of the diabetes.
Thus having a well coordination between the healthcare professionals in an interdisciplinary team the conditions of the diabetes mellitus can be effctively controlled.
