Creating a Plan of Care for a Client with a Reproductive Disorder

In this discussion, you are required to create a plan of care for Client Patricia, a 39-year-old who has a history of polycystic ovary disease and is currently overweight. She is ready to start a family and has been trying to conceive for fourteen months. Her husband has a teenage son from a previous relationship, but he would like at least one child with her. She is becoming frustrated when she gets her menstrual period.

The plan should contain one of the listed NANDA-I diagnoses. Please use your Mosby’s Guide to Nursing Diagnosis to help you write a complete plan. The plan should be written in the Nursing Diagnosis as 3-part nursing statement with the PES format:

  • E – Etiology is specified for nursing diagnosis and nursing practice can impact the problem
  • S- Signs & symptoms are supportive to diagnosis, accurate and specific to the client.

Please provide an explanation for the reason this NANDA-I should be addresses for this client- Consider what may happen to the client if this problem is not addressed. Please provide a rationale for using your chosen interventions, backed by evidence. You are required to provide a minimum of two interventions. Please provide a SMART outcome- (specific, measurable, attainable, realistic & timely) that would demonstrate the problem was successfully resolved.

  • Chronic sorrow
  • Ineffective health management
