So, I had a patient who is a 85 year old, no past medical or surgical history who came in after fall from 3rd floor of building during a suicide attempt. He contained multiple trauma, and hip fracture. Chest CT-scan showed right ninth rib fracture. Right wrist xray shows avulsion fracture noted at the dorsal carpus. Vital signs were stable, afebrile and room air. He has abdominal incision, staples were removed, dry, intact, no redness. Wound on the right elbow. PICC line was removed. Blood glucose was high 231. Pt is on diabetic diet, nutrition is decreasing, ate 15-25% of of brekafast and lunch. On foley catheter collecting light amber urine. No bowel movement since the 23rd. Physical therapy came in but refused to get up complaining of pain to hip and elbow. Foam boots to low extremities, incentive spirometer every 1 hr, SCD while in bed, turn and position every 2 hr.

Medications:

  1. Novolog Flx Pen 100 unit/mL 3mL Pen. Inj.
  2. Saline Lock Flush 10mL IV
  3. Colace capsule 100mg PO BID
  4. Cymbalta capsule 20mg PO daily
  5. Lovenox Inj. 40mg SQ Q24hr

  7. Protonix Inj. 40mg IV daily SCH
  8. Miralax Packet 17gm PO BID PRN

Received tetanus toxoid once only for the five pregnancies.

Prenatal checkup, twice at Health Center XXX

No family planning

Cannot recall childhood immunizations

Educational attainment: elementary graduate

Drinks liquor and smokes cigarettes prior to pregnancy.

Preferred foods: Fried Chicken and occasional junk foods with a bottle of coke

Mother has wrong perception about immunizations and did not practice breastfeeding with previous children.

Chief complaints upon admission: Mother is in severe abdominal pain, Cervical dilatation is 10cm and fully effaced. BOW already ruptured 2 hours prior admission.
