Therapeutic class : antidepressant
Pharmacological class : SSRI (selective serotonin reuptake inhibitors)
Escitalopram is used to treat depression and anxiety disorders.
Escitalopram works by Inhibiting reuptake of serotonin by binding to 5HT receptors.
While administering this drug nurse should keep a constant check on body weight of patient. Serotonin syndrome should be checked. A close eye should be kept on behavioural patterns and any suicidal attempts of patient. The drug should not be stopped abruptly, slowly tapper the dosage.
Adverse effects of escitalopram includes Nausea, headache, weight gain, tremor, sexual dysfunction, sleep disturbances, agitation, weight loss, constipation.
Escitalopram interacts with drugs like CYP inhibitors, dextromethorphan, medications that prolong QT interval, MAO inhibitors.
OVERDOSE TREATMENT
Mild cases- IV fluids for dehydration/hypotension, provision of benzodiazepines and avoidance of all serotonergic medications.
Moderate cases- hyperthermia is treated with cooling blankets, ice packs, and IV fluids. Activated charcoal. Benzodiazepine is given for neuromuscular abnormality.
Severe cases- all the above measures are used. Maintain airway through ventilation. Mechanical ventilator is used if needed.Treat with 5HT antagonists as cyproheptadine. short-acting agents amenable to titration, such as nitroprusside and esmolol are used for autonomic instability.
