1) wash your skin with warm water and tap dry it properly. Do not apply oil , perfume or any other product in that area otherwise it will become difficult to attach the pouch to the skin. Use skin care products for the healthy skin in that area. Remove hairs around that area so that pouch can easily attach.keep on checking the appearance of the surrounding skin.

2) 3 ways to prevent gastroenteritis- 1. Always wash fruits and vegetables before eating.2.always wash hands properly after using restroom and before food preparation.3. Don't share utensils or clothes with sick people.

3) stress causes worsening of IBD so it is very important to get involved in relaxation therapy. Nurse should recommend patient to breathe deeply and concentrate on the breathing pattern to become calm. The patient ahould not stay on bed all the time, for the initial few weeks it is difficult to manage stoma,but walking is the best way to exercise for these patients. Patient can perform yoga by securing stoma with the tight fitting layers.

4)nurse should check vital signs for any symptoms of hypotension,fever and tachycardia. Perineal care should be perform to prevent spread of infection. Collect and monitor lab reports of blood and stool sample to rule out any infection and to determine if supplementation is necessary. Promote oral care and bed rest. Medication should be administered on time.

5)laproscopic colectomy and restorative IPAA are becoming ideal option for the surgery. The surgery should be performed if medical therapy is not working. During the surgery it's important to maintain the safety of the patient. And also sterlization. If it is a early case it most probably end with terminal ileostomy. It is important to discuss the pros and cons of surgery with the patient too.

6) nutrition is the primary thing to avoid diverticulosis. If you eat high fiber diet when you have diverticulosis then it helps in softening of the stool and also decreases pressure in the colon and prevent flare ups.

7) ibd is an autoimmune disease so it is very important to keep check on the immune system. Should promote healthy habits and diet. It is important to ask patients to do exercise and to eat hygienic food and perform self hygiene. Nurse should give comfort to the patient by good communication skill. Maintainance of electrolyte balance is important. Rehydration solution should be administered at every few intervals of time.
