Support in various aspects of life positively affects coping strategies and influences the well-being and health outcomes of heart failure patients. Inadequate supportmay lead to a worsening of symptoms, increased hospital readmissions, psychological disorders, and a reduced quality of life.
Social support has been defined as resources (both tangible and emotional) that are provided by other persons for the aid of another. It has been depicted as comfort, assistance, and/or information that individuals receive through formal and informal connections with individuals or groups.Living alone has been reported to be associated with increased readmission rates in patients with chronic disease.
Primary,Secondary,and Teritiary Preventive needs:
The primary support system is from patient family,secondary support system from peer groups and teritiary support groups are doctors and health care staffs.

Specific,Measurable,Attainable,Relevant and time Bound objectives can improve CHF conditions for inadequate support system.
An Example : Encouraging daily walking behaviour through goal-setting, self-monitoring and providing feedback for people with CHF who are stable, knowledgeable and active increased heart failure related knowledge.Continued guidance of this Activity with SMART objectives can implement decreased hospitalisation and worsening of symptoms.
