Conduct an academic search of literature and identify current best practices associates with one nursing intervention for the person’s heart failure
Describe three strategies that activity involves the person to self-manage their Heart failure
Identify two supporting agencies/ health professionals that are currently contributing to the care of person’s individual needs (heart failure)
Nursing intervention:
Assessment of apical pulse , heart rate, rhythm to identify dysrhytmia- Rationale: usually tachycardia presents even at rest. Identifying arythmias help in managing the unstable conditions on time with appropriate mediactions
Monitoring BP- Rationale: In case of chronic heart failure, increased blood pressure will be there due to increased SVR, in advanced heart failure body is unable to compensate and leads to hypotension.
Monitoring urine output as patient may have reduced urinary output and concentrated urine.-Rationale: decreased cardiac output causing kidney for retention of sodium and water.
strategies of self management:
Increased time for rest
Active and Passive exercises
Using bedside commode for defecation inorder to avoid vagal stimulation thus preventing tachycardia and decreased cardiac output
Elevation of legs
Two supporting agencies/ health professionals
