Identify 2 nursing diagnoses, and appropriate nursing intervention for a client with Personality disorders
Nursing Diagnosis of Personality Disorders:
Diagnosed by factors like social isolation, impaired social interaction, high risk of violence to self and others, anxiety.
Physical Examination: Doctor physically examines the health related issues, this includes lab test for alcohol and drugs.
Psychiatric Examination: Done with the deep discussion about behaviour and feeling.
Diagnosing with criteria in the DSM-5, by comparing the symptoms by help of Diagnosis and Systemic Manual of Mental Disorders.
Nursing Interventions:
* Responding to the patient symptoms and needs.
* Patient require physical restraints, seclusion and supervision
* Keeping patient involved in treatment plan and to take responsibility of own behaviour and actions.
* Discussion with Patient and Family about the consequences and factors that triggering
* Patient can be treated by Cognitive analytical therapy, Cognitive therapy, Dialectic behaviour therapy (DBT), Dynamic Psychotherapy and Mentalisation Based Therapy (MBT)
