Describe a workplace you are familiar with.
For each type of hazard, describe an example of the hazard present
in the workplace, and what controls are used to mitigate the danger
associated with it. There should be an example of each type of
control (Engineering, Administrative, and PPE) for each hazard.
Example: Grocery Distribution Center
Temperature Hazard: -25° Freezer
Engineering Control: The freezer is walled off from the rest of the
perishable goods area
Admin Control: Personnel cannot work in the freezer more than 45
minutes at a time
PPE: Freezer suits, masks, gloves
Pressure Hazard: Ammonia lines for refrigeration
Engineering Control: Lines mostly run across the roof to keep them
away from employees
Admin Control: Only authorized personnel are allowed in ammonia
service areas
PPE: Enclosed eye protection, gloves, mask required when working on
ammonia lines
Electrical Hazard: Forklift battery
Engineering Control: Battery is enclosed inside forklifts, except
when being operated on by approved maintenance personnel
Admin Control: LOTO Program
PPE: Rubber gloves required when working on electrical
components
Fire Hazard: Flammable products (such as lighter fluid, roach
spray, etc)
Engineering Control: Aerosol room constructed inside the warehouse
to contain all flammable products; in the event of a fire in the
aerosol room, bay doors drop to enclose the fire.
Admin Control: All employees receive fire extinguisher and fire
escape training.
PPE: Fire response team has fire suits, SCBA gear.
Toxic Material Hazard: Battery acid (from industrial truck
batteries)
Engineering Control: Battery changing area located at far rear of
shop, beyond grocery picking area and away from non-maintenance
employees.
Admin Control: All employees perform pre-shift inspection of
battery caps before boarding truck (if truck allows access to
battery caps by non-maintenance personnel.)
PPE: Gloves, mask, goggles required for maintenance personnel when
working on battery
