obtained in Toua Xiong’s case to an actual patient care
situation.
1) The simulation went well. I felt after reading through the orders, i had a very clear idea of what needed to be done. This simulation went very well for me.
2) I realy felt that the thing that went the best was my confidence in knowing what to do and in what orders to do it. I felt i had a good grasp of what needed to be done for this patient after reading through the orders.
3) In all the vSims that i have completed, this is the only one that i have reaceived a perfect score on my first attempt. I really like that i felt confidence, that is not my normal emotion when going through the vSims.
4) Upon reading the scenario, the first problems mentioned were his difficult swallowing and the pain in his throat and mouth. After looking at patients chart and seeing what medications he was taking, i figured that the patient was dealing with a common side effect of oral inhaled corticosteroids thrush.
5) Underline cause for this patients problems were that he was taking an oral inhaled corticosteroids.
6) This patient is in need of teaching about proper use of his inhaled meds. He needs to be reminded to use his bronchodilator first, then followup with his corticosteroid. The dilation will allow more of the corticosteroid to get to his alveoli. After the patient is done inhaling his meds, he needs to swish his mouth out with water. He should also be using a soft bristle toothbrush so he is not being too aggressive when he is brushing his teeth and gums. He will need an antifungal medication to clearup his thrush, but doing those few things should help to reduce his symptoms and alleviate some of his discomfort.
7) Patients who have COPD have a number of valuable resources they can use to help them cope and deal with their disease. Some of the organizations available to help are Alfa-one Foundation, American Association for Respiratory Care, COPD International, The Global Initiative for Chronic Obstructive Lung Disease, American Lung Association, National Lung Health Education Program, Pulmonary Education and Research Foundation... I could go on. The point is there are numerous resources not only for patients, but for the family and caregivers of patients who suffer with COPD.
8) After going through the scenario, i feel that i gained valuble knowledge that will help me identify and help me to hopefully alleviate some of the adverse effects of patients who have oral health issued from medication. I look forword to being able to teach patients, and their caregivers tips and hints on ways to improve their lives even if they are suffering from chronic diseases. Little thing can mean a lot to a patient.
