Post a logic model and theory of change for a practitioner-level intervention. Describe the types of problems, the client needs, and the underlying causes of problems and unmet needs. Identify the short- and long-term outcomes that you think would represent an improved condition. Then describe interventions that would lead to a change in the presenting conditions. Be sure to search for and cite resources that inform your views.
Change is a continous process which can be measured in terms of behaviour or changes in the patterns of behaviour. Rather than types of problems , more challanging tasks is the representation of the problems . A problem may be described in various ways and the same words with different tones carries different meanings of the problems. At the practtioner level problems should be discussed with the level of high maturity and due care should be taken while elaborating the problems that the client should not get tensed or worried in any way.Time is an important factor and allocating more time to a client is another factor. Allocating more time to a patient is very helpful but its tough to maintain the professionalism in such scenerios thus with lesser time some inner thoughts which are undisclosed and thus are untouched or unmet.
Before interventions with clients a recording of clients expressions in forms of Cd or DVD may help both the parties to address the problems in more positive manner. After watching the recording , interventions would be much more helpful for the practioners as they can intervene the client directly and can touch those hidden thoughts which helps them to solve many critical problems which even clients are unable to understand better that lies within themselves.
