Identify a disease and explain how disease management might work within the context of the managed care concept. How could a managed care organization go about measuring utilization and/or costs for the selected disease?
Disease management is defined as a "system of medical
intervention and communication for the population, on condition
that patient self-care efforts are important."
For those who have access to a health professional or peer support,
this is a process where long-term individuals share knowledge,
responsibilities, and care plans with their health care providers
and / or friends. To be effective, it requires the full
implementation of community social support networks, professional
differences, and context-related activities, clinical professionals
wishing to partner or coaches, and online resources to be tested
and Compatible. Country and context, knowledge sharing, knowledge
building, and learning communities are an integral part of disease
management concepts. This is a wellness strategy for people as well
as a personal health approach. It can reduce health care costs and
/ or improve people’s quality of life by preventing or reducing the
consequences of diseases that are usually chronic through
knowledge, skills, and a sense of control over life and shared
care. On the other hand, this can increase the cost of healthcare
by incurring high costs of implementation and encouraging the use
of costly healthcare interventions.
