a nurse is monitoring a client during IV urography procedure. Which of the following client reports is the priority findings? a. metallic taste in mouth b. abdominal fullness c. feeling flushed and warm d. swollen lips
**Intravenous urography
An intravenous pyelogram (IVP), also called an intravenous urogram (IVU), is a radiological procedure used to visualize abnormalities of the urinary system, including the kidneys, ureters, and bladder. Unlike a kidneys, ureters, and bladder x-ray (KUB), which is a plain (that is, noncontrast) radiograph, an IVP uses contrast to highlight the urinary tract. An intravenous pyelogram may be used to diagnose disorders that affect the urinary tract, such as kidney stones, bladder stones, enlarged prostate, kidney cysts or urinary tract tumors.
During an intravenous pyelogram, you'll have an X-ray dye (iodine contrast solution) injected into a vein in your arm. The dye flows into your kidneys, ureters and bladder, outlining each of these structures. X-ray pictures are taken at specific times during the exam.
Nursing management
The nurse must monitor the patient throughout the procedure. For IV urography complication are rare.
The risks include
1.mettalic taste in mouth and
2.A feeling of warmth or flushing.
The nurse must give first priority to this two complications. This is due to the injection of x ray dye.mainly used( iodine contrast solution )
Rarely, severe reactions to the dye occur, including:
Extremely low blood pressure
A sudden, full-body allergic reaction that can cause breathing difficulties and other life-threatening symptoms (anaphylactic shock)
Cardiac arrest
During the X-rays, you'll be exposed to low levels of radiation. The amount of radiation you're exposed to during an intravenous pyelogram is small, so the risk of any damage to cells in your body is extremely slow.
-Nurse must ask questions about your medical history.
-Check blood pressure, pulse and body temperature
-provide hospital gown and remove jewelry, eyeglasses and any metal objects that may obscure the X-ray images
-Place an intravenous (IV) line into a vein in your arm through which the X-ray dye will be injected
-Instruct the patient to urinate to ensure your bladder is empty for the exam.
