Answer : Dobutamine brand name : Dobutrex. Generic name : Dobutamine   Type of drug : inotropic agents Pregnancy category : FDA pregnancy category B Dobutamine is a class of drug that stimulates heart muscle and improves blood flow by helping the heart pump better. Dobutamine is used short -term to treat cardiac decompensation due to weakened heart muscle .Therapeutic action : Dobutamine directly stimulates beta -1 receptors of the heart to increase myocardial contractility and stroke volume, resulting in increased cardiac output . In human urine , the major excretion products are the conjugates of dobutamine and 3-0-methyl dobutamine . Indications : 1) heart failure due to cardiac surgery, septic or cardiogenic shock 2) Dobutamine can be used for increasing cardiac output in the case of congestive heart failure . 3) for correction of hemodynamic imbalances present in the shock syndrome due to myocardial infarction ,trauma , renal failure, endotoxic septicemia and others . Contraindications : 1)Dobutamine is contraindicated with patients with a history of idiopathic hypertrophic sub-aortic stenosis . 2) patients with history of allergic -reactions .The other conditions such as 3) low amount of potassium in the blood 4) high blood pressure 5) ventricular premature beats 6) hypertrophic cardiomyopathy 7) atrial fibrillation . Dosage :intial dose of adult :  0.5 to 1 mcg/kg/mt . Maximum dose : 40 mcg/kg/mt . Always decide dosage according to blood pressure, cardiac output , renal condition and other conditions. Pediatric dosage : initial dosage : 0.5 to 1 mcg/kg/wt .Maximum dose : 40mcg/kg/mt but it is not recommended for children under 30 kg weight.
