A.) The following nursing care can be provided to this young lady if she is pregnant.
a) nausea and vomiting: The nursing care includes,
-advise the client to munch some toast/rusk before arising
-providing small and frequent meals throughout the day
-providing fluids between meals to prevent the fullness of the stomach
-avoid an empty stomach
-avoid fried foods
-avoid strong and pungent smell foods for the client
-provide food of her choice
b)Feeling tired of fatigue: The nursing care includes,
-advise the mother to take rest whenever she feels tired
-provide a client healthy and balanced diet
-advise the mother to take 2 hours afternoon nap every day
-advise the mother to avoid stress
-advise the mother to avoid strenuous activities
-light and moderate activities only can be advice
Apart from the above nursing care, the other intervention can also be given such as,
-proper immunization advice such as TT vaccine
-iron and folic acid supplementation
-educate clients on warning signs of hypertension and other complications
-educate the client on how to prevent constipation and heartburn
-educate the client on avoiding smoking, consumption of alcohol, and avoiding radiation
