Individualised support plan helps in progressing towards health , minimises the complications, enhances the quality of health life which results in happiness and productivity in one's life. This is done by support health worker keeping in mind how the client wants to lead his life which also includes support system like families, neighbourhood, friends, community.

Impact of key changes in individualised support plan :

• The client may be initially confused with changes.
• The client may be reluctant to implement at first or may be happy with the changes.
• The client may start liking the changes if he is benefited from it or vice versa.
• The client may be discouraged with the whole plan and the motivation may seem lost depending on the personality of the client or vice versa.
• The client may feel off the balance in keeping record of the plan and implementation of the plan.
• Even for the health care worker , it becomes difficult to monitor and evaluate the plan after the key changes.
• Changes may seem difficult with people who has certain personality disorders.
