  • Avoid asking or forcing the client to make choices. Use short, simple directions.

  • Help client set realistic goals.
  • Assist client to meet activities of daily living
  • Promote more rest and sleep
  • Encourage client to meet hygienic need
  • Place the things near to the client

3. Self-Care Deficit related to excessive ritualistic behavior as evidenced by unwillingness to perform self-hygiene.

Nursing interventions

  • Encourage independence to client.
  • Offer recognition and positive reinforcement for independent activities
  • Show client how to perform activities with which he is having difficulty with.
  • Keep strict records of food and fluid intake
  • Assist client to take food and other activities
