progression of condition, and treatment regimen as evidenced by
feelings of discomfort, apprehension or helplessness .

- Maintain a calm, non threatening manner while giving care to
client.
- Establish and maintain a trusting relationship with the
client
- Remain with the client whenever neede
- Educate the patient that anxiety disorders are treatable.
- Avoid asking or forcing the client to make choices. Use short,
simple directions.
2. Powerlessness related to consistent negative feedback as
evidenced by verbal expressions of having no control

- Assist client to meet activities of daily living
- Promote more rest and sleep
- Encourage client to meet hygienic need

3. Self-Care Deficit related to excessive ritualistic behavior
as evidenced by unwillingness to perform self-hygiene.

- Encourage independence to client.
- Offer recognition and positive reinforcement for independent
activities
- Show client how to perform activities with which he is having
difficulty with.
- Keep strict records of food and fluid intake
