nursing care plan for Linda Pittmon, a 74 -year old female patient who is a noncompliant diabetic, and frequently stays at the local homeless shelter. She has been admitted to the floor with complaints of numbness in her right foot and ankle. Mrs. Pittmon states she has had numbness for years but “now I can’t feel it at all, and my toes don’t look the right color.”. The care plan must have 5 priority nursing diagnosis.
1. Disturbed sensory perception related to hyperglycemia.
Protect the patient from injury.
Protect the patient from exposure to cool drafts or hot water.
Assist the patient with ambulation for patient safety.
2. Impaired skin integrity related to peripheral neuropathy .
Assess the integrity of skin.
Advise the patient not to walk barefoot.
Inspect the feet daily .
3 . Risk for injury related to numbness of foot .
Assess the appearance of the foot.
Advise the patient to inspect the feet for cuts , scratches and blisters.
Advice the patient to wear protective footwear.
4 . Risk for unstable blood glucose related to lack of acceptance of diagnosis.
Assess for signs of hyperglycemia.
Monitor HbA1c-glyvosylated haemoglobin.
Assess the patient's knowledge about prescribed diet.
5. Deficient knowledge
Explain the patient about insulin dosages.
Teach the patient about the diet .
Teach the patient about the signs of hypoglycemia
