Create A NURSING CARE PLAN FOR A CHILD DIAGNOSED WITH

Nursing care plan :the nursing care plan provide a direction for individualized patient care and provide continuity of care for the patient with all departments, a) Immunodeficiency disorders on children: the immunodeficiency disorders associated with defect or impairment in immune fuctions or induced through infections and various environmental factors,it may be due to congential or result of other disease, so in this nursing care plan diferes from one type of immunodeficiency disorders and it includes immunodeficency syndrome, chronic fatigue and immune dysfunction syndrome, and severs combined immunodeficency diseases etc , the nursing care plan includes diagnoisis, objectives of care and nursing interventions and rationale,a) diagnosis includes : activity intolerances, distrubed body image, fatigue , hyperthermia, imbalanced nutritions, impaired skin integrity, tissue integrtiy, interrupted family processes, risk for deficiencies, risk for infections, social pressures etc, b) objectives of care include: the patient will verbalize the importances of activity, and energitic, and have good nutrition, and the family will support the patient and the patient will comply with treatment regimen, etc c) nursing intervention and rationale:montior the patient for fever, fatigue, tender lyphnodes etc and check laboratory values regularly, and record the patient calorie intake , and nutritious food, and ensure adequate fluids intake, and encourage the patient to maintain good spirit and postivenenss and much physical activity and monitoring any infections and giving treating the infections orderly, and giving medication according to the symptoms, and educating the parents and family about disease transmission or congential and prevention . 2)Kawasaki disease or mucocutaneous lymph nodes syndrome : it is an acute febrile vasculitis syndorme of early chilhood , the nursing care plan includes a) assessment : the child is assessed in every phase of the disease , b) nursing diagnosis: based on the assessment data the major nursing diagnosis are chronic pain, risk of decreased cardiac output, activity intolerance, impairedskin integrity, impaired oral mucous membrane, c) nursing planning and goals: the patient and family will monitor the child closely for any inflammation, and the need for possible oximetry, and the patient and family will know the medications used to treat inflammation and understanding the need for echocardiogram and possible EKG for cardiac status and the patient and family will follow coping strategies and follow the plan evne at home d) nursing intervention: montiro pain, cardiac monitoring and assessment and monitor intake and output, and plan periods of rests and activities and provide oral care e) evaluation : child symptoms present imporving and overall child condition is improved, fever is not there for 18hours prior to discharge, echocardigram is complete, cardiologist has seen child, and given follow up appointment and childs physician has been contacted.
