Team work and collaboration are especially essential to care of patients in a decentralized health system with many levels of health works.Team can also work together to develop health promotion for diverse communities and instill disease prevention behaviors amongst patients.

Health care oraganization team members including an attending physician,a resident,a nurse practitioner or physician's assistant,nurse manager,nurse, patient care technician,social worker, dietitian, therapist, home care liaison nurse etc.

Dysfunctional roles which includes,

1. Absence of trust

The fear of being vulnerable to team members prevents the building of trust within the team,This occur when team members are reluctant to be vulnerable with one another and are unwilling to admit their mistakes, weaknesses or needs for help without a certain comfort level among team members,a foundation of trust is impossible.
